We are looking for an enthusiastic Clinical Nurse Consultant (Cardiac) to join our team.
Full-time Fixed Term: 12 moths' engagement
Location: Melbourne OR Brisbane – Virtual role
As part of the SVVH division, you’ll play a vital role in shaping the future of virtual and home-based healthcare. Hearts at Home is a new initiative supporting individuals living with heart failure, enabling early discharge from hospital and reducing avoidable admissions. You’ll provide high-quality, evidence-based clinical assessment, care planning, intervention, education, and management—leveraging virtual care and remote monitoring technology to make a real difference in people’s lives.
You’ll work closely with hospital and primary care teams, using your expertise to coach clients in self-management, coordinate care, and ensure seamless transitions. Your commitment to continuous improvement, innovation, and compassionate care will help drive service development and enhance outcomes for our community.
About You:
We’re looking for candidates with at least 5 years’ post-registration nursing experience (including 3 years in cardiac care, chronic disease management, or critical care), a strong background in digital health, and a passion for new models of care.
