Job Description
We are looking for a healthcare professional with a solid understanding of medical terminology and ICD-10 coding who is ready to step into a quality-focused role reviewing clinical documentation for accuracy and compliance to join our team as a
Clinical Documentation Auditor / Medical QA Specialist
. This role is ideal for someone who thrives on structure, precision, and supporting clinical teams behind the scenes.
Responsibilities
- Audit and review clinical documentation in the EMR system for completeness, accuracy, and regulatory compliance
- Confirm appropriate use of functional status assessments (0–6 scale) and ensure diagnosis codes are accurately applied
- Ensure ICD-10 codes are consistent across referrals, OASIS forms, and clinical progress notes
- Track and monitor documentation timelines, adhering to the 5-day window for longer forms and 3-day window for shorter ones
- Follow up with clinicians regarding incomplete, delayed, or incorrect documentation
- Communicate with physician offices to request missing or corrected paperwork
- Verify alignment of documentation with CMS quality standards and insurance requirements
- Review medication lists, diagnoses, and clinical notes to ensure data integrity and compliance
- Support internal audits and help maintain complete, accurate, and up-to-date patient records
- Maintain open communication with clinicians using WhatsApp and ensure timely documentation follow-ups
Requirements
- College educated
- Candidates must provide either an NBI Clearance or Police Clearance (preferred) upon hiring
- Candidates must provide a proof of separation from their previous/current employer with an indication/confirmation of their last date of employment
- Must be comfortable with using Hubstaff, a time-tracking application
- Professional-level English (written and verbal/voice)
- Strong understanding of medical terminology and ICD-10 coding
- Excellent computer skills and ability to quickly learn EMR systems
- Experience in quality assurance, documentation review, or chart auditing
- Clinical background in nursing or healthcare is a plus, but NOT required
- Familiarity with home health documentation and CMS standards
- Highly detail-oriented with excellent organizational skills
- Strong written and verbal communication abilities
- Proficiency in using Kinnser (EMR system), Waystar (insurance eligibility & verification, MyFacts (electronic faxing), WhatsApp (for clinician communication), Google Workspace (Docs, Sheets, Drive, etc.)

- A noise-canceling headset
- At least a 25mbps primary internet connection
- A backup laptop, backup internet connection device, and backup power supply in case of power outage and emergencies
- Minimum recommendation for your main computer: Intel Core i5 or equivalent with 8 GB RAM or higher
- Minimum recommended for your backup computer: Intel Core i3, 4 GB RAM
