What You’ll Do

• Conduct Medicare Annual Wellness Visits

• Complete risk adjustment documentation

• Close HEDIS (quality measures) care gaps

• Perform HCC and ICD-10/CPT II coding

• Use EMR tools for efficient visit completion

Must-Haves

• Active Indiana NP license

• 1+ year in Family or Internal Medicine

• Enrolled with Medicare and Medicaid

• Familiar with Medicare, Medicaid, and Commercial plans

• Reliable internet and private workspace
