GOBHI has an immediate opening for a full-time Clinical Care Coordinator and we are flexible on the home office location within the State of Oregon. The Clinical Care Coordinator will provide high quality, integrated, trauma informed and culturally competent care coordination services to members who have complex behavioral health care needs. These may include medical, substance use and any other comorbidity from dental to dementia.
The Clinical Care Coordinator is a key position with a primary purpose to make sure members’ needs are met by developing plans of care. This position works directly with community based providers of patient care, including; hospitals, Community Mental Health Providers (CMHP), allied agencies, residential and primary care clinicians in addition to peer providers and attachment figures. The position also cross covers in the Utilization Management (UM) role and necessitates the ability to work after hours and weekends on occasion.
