Nurse Practitioner (NP)

Chaim Gitelis DO PC is seeking a compassionate and experienced Nurse Practitioner (NP) to provide care inside assigned nursing facilities while supporting Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and Behavioral Health Integration (BHI) programs.

This role is primarily onsite, with limited remote flexibility for documentation or follow-ups when appropriate.

• Provide onsite patient assessments, follow-up visits, and chronic-care evaluations
• Manage CCM, RPM, and BHI services for residents
• Document all encounters within 48 hours
• Create and update individualized patient care plans
• Identify and close gaps in care
• Escalate patient concerns to supervising providers when needed
• Collaborate with facility nursing leadership and interdisciplinary teams
• Ensure compliance with CMS guidelines, HIPAA, and internal policies
• 8–12 patients/day, rounding + follow-ups(adjustable based on need)

• Active Nurse Practitioner license (or willingness to obtain additional required state licenses)
• Florida APRN license required (we reimburse licensing costs).
• Experience in geriatrics, long-term care, chronic disease management, or primary care
• Proficient with EMRs and digital documentation
• Strong communication and clinical assessment skills
• Reliable, organized, and comfortable working independently in a facility setting

Schedule

• Monday–Friday
• 9:00 AM – 5:00 PM EST
• Primarily onsite at assigned facilities, with some remote flexibility
