Population Health Nurse Coach

We're seeking a Certified Health Coach and Registered Nurse who is passionate about behavior change, member engagement, and helping individuals overcome the barriers that prevent them from achieving better health.

This is not a traditional case management role.

Our most successful Nurse Coaches build trust quickly, use motivational interviewing with confidence, address behavioral and emotional barriers to change, and help members take ownership of their health journeys.

You'll work closely with members managing chronic medical conditions and behavioral health challenges, guiding them toward sustainable lifestyle changes that improve outcomes and quality of life.

What You'll Do: Your Roadmap to Impact

In this role, you'll serve as a trusted health partner and coach, helping members create meaningful and sustainable change.

Deliver High-Impact Health Coaching

  • Conduct telephonic and virtual coaching sessions using motivational interviewing and evidence-based coaching techniques.
  • Help members identify barriers, increase readiness for change, and establish achievable health goals.
  • Guide members toward sustainable improvements in physical health, emotional well-being, and lifestyle behaviors.

Support Members with Complex Needs

  • Coach individuals managing chronic medical conditions and co-occurring behavioral health challenges.
  • Recognize how stress, anxiety, depression, motivation, resilience, and social circumstances affect health outcomes.
  • Partner with members to develop realistic strategies that support long-term success.

Build Engagement and Accountability

  • Establish trust-based coaching relationships that encourage participation and follow-through.
  • Re-engage members who have become disengaged or resistant to care plans.
  • Maintain regular follow-up and accountability conversations that reinforce progress and celebrate success.

Coordinate Care and Advocate for Members

  • Collaborate with providers, behavioral health professionals, and internal clinical teams.
  • Connect members with resources, programs, and services that support whole-person care.
  • Help members navigate healthcare systems and remove barriers to care.

Measure Progress and Outcomes

  • Document coaching interactions and care plans accurately.
  • Track member engagement, behavior change goals, and health outcomes.
  • Use data and coaching insights to continuously improve member success.

Who May Not Be a Good Fit

This role may not be the best fit for individuals who:

  • Prefer short-term interactions instead of developing longitudinal coaching relationships
  • Become frustrated working with individuals who are hesitant, resistant, or slow to change
  • Prefer directing patients what to do rather than partnering with them to discover solutions
  • Are uncomfortable discussing behavioral health challenges, emotional barriers, stress, motivation, or readiness for change
  • Prefer highly structured workflows with limited outreach, follow-up, and relationship building

Who You Are: The Nurse Coach We're Looking For

Certified Coach You hold a Health Coach Certification from a recognized coaching organization and actively apply coaching methodologies in your practice.

Behavioral Health Experience You have experience supporting individuals with behavioral health conditions, emotional wellness concerns, or behavior-related barriers that impact health outcomes.

Experienced Nurse You possess an active, unrestricted RN license and experience working with chronic disease management, population health, health coaching, care management, or wellness programs.

Expert in Behavior Change You understand that sustainable health improvements require more than education. You are skilled in motivational interviewing, stages of change, goal setting, and member activation.

Relationship Builder You connect with people quickly, build trust, and foster meaningful coaching relationships in a virtual environment.

Strong Communicator You can translate complex clinical information into practical, actionable steps that members can understand and implement.

Independent and Accountable You manage your member panel effectively, stay organized, and consistently follow through on commitments.

Required Qualifications

  • Active, unrestricted RN license
  • Health Coach Certification (required)
  • 3+ years of experience in population health, chronic disease management, health coaching, care management, or wellness coaching
  • Demonstrated experience supporting individuals with behavioral health needs
  • Experience utilizing motivational interviewing and behavior-change methodologies
  • Strong verbal communication and relationship-building skills

Preferred Qualifications

  • National Board Certified Health & Wellness Coach (NBC-HWC)
  • Behavioral Health Coaching Certification
  • Experience working with anxiety, depression, stress management, resilience, and lifestyle behavior change
  • Diabetes Prevention Program (DPP), CDC, or chronic disease coaching experience
  • Telephonic or virtual coaching experience

Mental and Physical Requirements

• This is a fully remote role requiring a HIPAA-compliant home workspace
• Work is primarily sedentary with prolonged computer and phone use
• Requires sustained focus, active listening, and continuous communication throughout the day
• Ability to manage multiple members, priorities, and documentation requirements in a fast-paced environment
