Day in the Life of a Telehealth Nurse Practitioner

As a Telehealth Nurse Practitioner (NP) at Crossroads, you’ll play a pivotal role in delivering life-changing care to individuals battling substance use disorders. Using cutting-edge virtual platforms, you’ll assess, diagnose, and manage substance use conditions — with a focus on opioid use disorder (OUD). You'll prescribe buprenorphine and other MOUDs in accordance with regulatory guidelines, working under a collaborative practice agreement to ensure patients receive accessible, expert care wherever they are.

Key Responsibilities

  • Conduct comprehensive telehealth visits, including intake assessments, follow-up appointments, and annual evaluations for patients with SUD.

  • Diagnose and manage opioid use disorder and co-occurring conditions using MOUD, including buprenorphine.

  • Review and interpret urine drug screens, prescription monitoring data, and laboratory results to guide clinical decisions.

  • Collaborate with behavioral health providers, counselors, and care coordinators to support integrated treatment plans.

  • Educate patients on MOUD, recovery planning, harm reduction, and relapse prevention strategies.

  • Document all patient encounters accurately and timely in the electronic health record (EHR).

  • Participate in case reviews, supervision, and ongoing training as required.

  • Maintain compliance with DEA regulations, state practice laws, and Crossroads protocols.

Schedule & Locations

  • Target weekly hours: 40 hours weekly

  • Sample schedule: Monday- Friday 8am-5pm, possible weekends

  • This role will provide coverage throughout PA initially and then other states as needed.

Education and Licensure Requirements

  • Active and unrestricted Nurse Practitioner license in the state(s) of practice.

  • Certification as a Family (or Adult) Nurse Practitioner (FNP) or Psychiatric-Mental Health Nurse Practitioner (PMHNP) preferred.

  • Active DEA license with appropriate credentialing to prescribe buprenorphine under MOUD treatment standards.

  • Minimum of at least 1 year experience treating patients with SUD or in behavioral health preferred.

  • Prior telehealth experience is a plus.

  • Proficiency with EHRs and comfort with virtual care platforms.
