Position Summary:
The Virtual Care Center is a means of providing rapid access for patients who have an acute medical condition and need a same day appointment with a clinician using telehealth. The Virtual Care Center Registered Nurse supports prioritizing acute patients, ensuring patients are seen in a timely manner, monitor the provider's template to facilitate patient flow.. The RN is instrumental in ensuring our patients receive quality care via telemedicine.
This is a remote (work from home) position, however incumbents must be comfortable with coming onsite in Bronx based locations as needed.
Minimum Qualifications:
- Bachelor's Degree in Nursing is required.
- NYS Licensed Registered Professional Nurse.
- 1-2 years of direct clinical experience in a primary care environment working with adults and children.
- Bilingual English/Spanish required.
- Previous experience/expertise using electronic health records is a must.
- Ability to thrive in a telemedicine environment.
Key Duties:
- Triages patients based on acuity and ensure they are seen within a timely manner.
- Monitors call center template for pediatric and adults and discharged patients.
- Management of abnormal labs/DI and critical values.
- Answers clinical calls from variety of settings, including hospitals, pharmacy, department of health, and call center agents.
- Follows up with RPM patients to assess acuity based on vitals.
- Assists Virtual Care Center Medical Assistants with clinical guidance and support.
- Flows patients to providers throughout the organization to ensure patients are prioritized based on acuity.
- Other duties as assigned by Director of Virtual Care Center.
Schedule: M,Th, Fri 1pm-10pm; Tu 10am-6pm, Wed, 8am-6pm. Rotating Saturday shifts 8a-6pm with Wednesdays off.

Compensation: Minimum $82,560/annum; Maximum $98,560/annum
