RN Surveyors are responsible for conducting onsite healthcare facility surveys to evaluate compliance with federal healthcare regulations and standards governing patient care, resident safety, infection prevention, nursing services, and operational quality.

Responsibilities

This position is fully remote with travel requirements up to 75% nationwide.
The Registered Nurse (RN) Healthcare Facility Surveyor will function as either a survey team member or team lead supporting a variety of healthcare facility surveys and regulatory investigations. Survey types may include recertification surveys, complaint investigations, comparative reviews, follow-up visits, and monitoring surveys involving long-term care and non-long-term care healthcare settings.
Facilities surveyed may include:

  • Skilled Nursing Facilities (SNFs)
  • State Veterans Homes (SVH)
  • Community Living Centers (CLC)
  • Ambulatory Surgical Centers (ASC)
  • Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/IID)
  • End-Stage Renal Disease (ESRD) Facilities
  • Psychiatric Residential Treatment Facilities (PRTF)
  • Hospitals
  • Critical Access Hospitals
  • Hospice Facilities

RN Surveyors travel nationally to conduct onsite reviews evaluating compliance with federal healthcare regulations and standards related to patient care, nursing services, resident safety, and facility operations.
Responsibilities include:

  • Conducting onsite surveys of Medicare and Medicaid certified healthcare facilities to evaluate compliance with applicable federal regulations and standards
  • Preparing for surveys through offsite review activities, including review of facility documentation and regulatory history
  • Performing facility tours, resident observations, medical record reviews, and interviews with staff, residents, and family members
  • Assessing whether healthcare services, treatment practices, and resident care activities are consistent with federal requirements and accepted standards of care
  • Evaluating multiple operational areas, including:
    • Nursing services
    • Infection prevention and control
    • Resident rights
    • Quality assurance activities
    • Physical environment conditions
    • Clinical documentation
    • Medication administration practices
  • Identifying deficient practices and documenting noncompliance findings accurately and thoroughly
  • Discussing survey findings and deficiencies with facility leadership throughout the survey process and during exit conferences to ensure understanding of identified concerns
  • Reviewing facility plans of correction and determining whether corrective actions appropriately address identified deficiencies
  • Conducting follow-up surveys and monitoring visits to verify implementation of corrective actions and sustained compliance
  • Preparing detailed written reports and supporting documentation within required timelines
  • Utilizing CMS and VA survey systems including ASPEN and IQIES
  • Assisting with internal training activities, facility education, and participation in workgroups or quality improvement initiatives as assigned

Minimum Qualifications

  • Associate’s Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) required
  • Active, unrestricted Registered Nurse (RN) license required
  • CMS Long-Term Care Basic Training and successful completion of the Surveyor Minimum Qualifications Test (SMQT) preferred
  • Minimum two (2) years of recent experience conducting surveys, accreditation reviews, or compliance inspections involving long-term care or healthcare facilities
  • Demonstrated ability to independently manage survey responsibilities and effectively lead or participate in survey activities
  • Strong analytical, communication, interviewing, and written documentation skills
  • Ability to independently prioritize assignments and collaborate effectively within multidisciplinary survey teams
  • Ability to professionally discuss regulatory findings, address conflict, and seek guidance appropriately when necessary
  • Ability to work remotely in a HIPAA-compliant environment
  • Ability and willingness to travel nationwide up to 75% of the time

Preferred Qualifications

  • Prior VA, CMS, or state survey agency experience
  • Experience with:
    • ASPEN
    • IQIES
    • Long-term care compliance surveys
    • Federal healthcare regulations
    • Immediate Jeopardy (IJ) investigations
  • Experience reviewing:
    • Infection control programs
    • Medication management
    • Resident care planning
    • Clinical quality indicators
    • Wound care documentation

Compensation

Estimated compensation range:

  • $82,000 – $95,000 annually
