POSITION PURPOSE


The Special Projects Registered Nurse (RN) supports organizational clinical initiatives that improve quality outcomes, member engagement, medication adherence, preventive care, and operational efficiency. The role partners closely with the Director of Clinical Services on strategic projects, quality initiatives, clinical outreach, point-of-care testing follow-up, and operational improvement efforts. 


DUTIES & RESPONSIBILITIES


Clinical Outreach & Member Engagement

  • Conduct outbound calls to members for medication adherence initiatives.
  • Educate members regarding medication compliance, refill needs, and barriers to adherence.
  • Collaborate with pharmacies, providers, and health plans to improve medication adherence rates.
  • Perform member outreach related to preventive care opportunities, quality measures, and health education.
  • Assist members in scheduling appointments and obtaining recommended preventive services.
  • Document all member interactions accurately within the electronic medical record.

Clinical Quality & Population Health

  • Assist with clinical quality improvement projects designed to improve HEDIS, STAR, NCQA, URAC, and contractual performance measures.
  • Review medical records and identify opportunities for care gap closure.
  • Support annual quality initiatives and population health campaigns.
  • Assist with chart abstraction, medical record review, and quality reporting.
  • Participate in audits and quality improvement activities.
  • Monitor assigned project metrics and provide progress updates to leadership.

Point-of-Care (POC) Testing Support

  • Assist the Director of Clinical Services in reviewing Point-of-Care (POC) testing results.
  • Notify members of normal and abnormal POC test results following established clinical protocols.
  • Coordinate follow-up appointments, laboratory testing, or provider referrals when indicated.
  • Ensure timely documentation and communication of testing outcomes.
  • Escalate abnormal findings to the appropriate provider or clinical leadership.

Clinical Project Management

  • Support implementation of new clinical programs and operational initiatives.
  • Coordinate assigned clinical projects from initiation through completion.
  • Track project milestones, deliverables, and timelines.
  • Develop project documentation and workflow recommendations.
  • Identify barriers and recommend process improvements.
  • Collaborate with interdisciplinary departments to ensure successful implementation of projects.
  • Assist in developing policies, procedures, and standardized workflows.

Clinical Operations Support

  • Serve as a clinical resource for operational departments as assigned.
  • Assist leadership with implementation of new clinical workflows.
  • Participate in interdisciplinary meetings, committee work, and project planning sessions.
  • Assist with staff education related to new clinical initiatives.
  • Monitor assigned work queues and reports.
  • Assist leadership with data collection and analysis for clinical initiatives.

Compliance & Documentation

  • Maintain accurate, timely, and complete documentation within the electronic medical record.
  • Ensure compliance with HIPAA, NCQA, URAC, CMS, state regulations, and company policies.
  • Maintain confidentiality of protected health information.
  • Report quality concerns, patient safety issues, or compliance concerns to leadership.
  • Perform other duties as assigned.

REQUIRED KNOWLEDGE, SKILLS & ABILITIES

  • Strong clinical assessment skills.
  • Excellent verbal and written communication.
  • Ability to manage multiple projects simultaneously.
  • Strong organizational and time management skills.
  • Knowledge of population health management principles.
  • Knowledge of medication adherence initiatives and quality improvement programs.
  • Ability to work independently while collaborating across multiple departments.
  • Strong problem-solving, critical-thinking, and attention to detail.


MINIMUM QUALIFICATIONS

  • Active Registered Nurse (RN) license in the state(s) of Arizona, Louisiana, Nevada.
  • Minimum of two (2) years of clinical nursing experience.
  • One (1) year of case management, care coordination, quality management, utilization management, population health, or disease management experience preferred.
  • Experience with HEDIS, STAR, NCQA, URAC, or CMS quality programs preferred.
  • Microsoft Office proficiency.


PREFERRED QUALIFICATIONS

  • Certified Case Manager (CCM) preferred.
  • Population Health experience preferred.
  • Quality Improvement experience preferred.
  • Project management experience preferred.
  • Bilingual English/Spanish preferred.


LICENSES & CERTIFICATIONS

  • Current Registered Nurse (RN) license in AZ, LA, NV
  • Current CPR certification.
  • Current TB screening (if applicable).

PHYSICAL DEMANDS & WORK ENVIRONMENT

  • This position involves a combination of office-based work and fieldwork within the community.
  • Travel will be required to attend meetings, conduct home visits, or participate in community events.
  • Must be able to frequently drive long distances, which may require a two-hour radius of your designated county (travel time is paid and mileage reimbursement is available).
  • Occasionally required to stand.
