Telehealth Nurse Practitioner (On-Call) - Remote KY/WV/OH

Telehealth Nurse Practitioner (APRN) for our Skilled Nursing and Post-Acute Facilities - Remote, covering Kentucky, West Virginia, and Ohio

Compensation: $105,000 - $125,000 Annual Salary

We are seeking a Nurse Practitioner to provide telehealth coverage for our skilled nursing and post-acute facilities across Kentucky, West Virginia, and Ohio. This role can be full-time or part-time and includes on-call coverage during nights or weekends. We are offering COMPETITIVE PAY with AMAZING BENEFITS.

Qualifications:

• Active APRN license in Kentucky, West Virginia, and Ohio
• Active DEA registration
• Comfortable with telehealth visits and after-hours on-call coverage
• Skilled nursing, long-term care, or geriatric experience preferred

Responsibilities:

• Conduct telehealth visits and manage acute changes in condition after hours
• Provide on-call coverage during nights or weekends
• Document thoroughly and timely in the EMR
• Coordinate with on-site nursing and provider teams
